Healthy Start Team Members

Services Provided 

  • Home visiting support and education for a healthy pregnancy
  • Connection to services requested and identified 
  • Education: parenting, breastfeeding, immunizations, planning future pregnancies, and healthy eating
  • Depression screening and referral
  • Fatherhood support 
  • Infant assessment and referral
  • Help to quit smoking and reduce stress

Services are available to all Duval County pregnant women and children up to three years old (women who are in between pregnancies are also eligible).

Eligibility Criteria 

Any pregnant woman can take the Florida Healthy Start Prenatal Risk Screen, a short questionnaire, which is offered by medical providers.  The screen is offered at doctor’s offices and birthing hospitals.

Service Locations 

Nurses and counselors provide education and support primarily in home or over the phone.  The team can also provide services to meet a family’s need at places in the community.

Healthy Start Services are free of charge

The Northeast Florida Healthy Start Coalition, Magnolia Project, Azalea Project, UF Health, Children’s Home Society, Healthy Families, The Department of Children & Families, and Early Head Start.

How Can I Enroll? 

Contact your healthcare provider or Healthy Start and ask to fill out a Healthy Start screening form. The form is a short questionnaire and your answers are confidential.
