Death certificates are available for deaths occurring in the state of Florida from 2009 to present. Any death prior to 2009 must be requested from the Florida Department of Health’s Bureau of Vital Statistics in Jacksonville or through VitalChek®. Orders may be placed in-person, by mail, or online.
Obtain a Copy in-Person or by Mail
Visit (or mail your request to) our Vital Statistics office. We are open Monday through Friday from 8 a.m. to 5 p.m.
You will need the following information:
- Legible copy of a valid driver’s license, passport, state-issued ID, or military identification card must accompany all requests.
- Completed Death Certificate Application Form should be submitted along with the appropriate fee.
- You must be a family member or legal representative when requesting a certificate listing the cause of death.
Fees
- $15 – death certificate
- $10 – additional certificates
