Please note that our office only has access to Florida vital records. If your birth occurred in another state, you may use our third‑party vendor, VitalChek, to complete your request.
Order in-Person or by Mail
Visit (or mail your request to) our Vital Statistics office. We are open Monday through Friday from 8 a.m. to 5 p.m.
You will need the following information:
- A valid form of photo identification such as a driver’s license, state-issued ID, passport, or military ID. All identification must be valid (cannot be expired.) For mailed requests, please provide a clear, readable photocopy. If you do not have a valid photo ID, call 904-253-1620 and we will provide 3 alternative documents that you can use.
Fees
- $15 Birth Certificates
- $10 Additional Certificates
- You will also need to complete a Birth Certificate Application, including the person’s full name, date of birth, county of birth, mother’s full name, including maiden name and father’s full name.
Curbside service is available for birth certificates. To use this service:
- Complete the Birth Certificate Application.
- Email the completed application to DuvalVitalStatisticsInfo@FLHealth.gov with valid ID.
- Use “Curbside” as the subject line of your email.
- A staff member will contact you shortly after your application is received for payment and pick up time.

Birth certificates in Florida prior to 1930 must be requested from the Florida Department of Health’s Bureau of Vital Statistics in Jacksonville. - Birth Records Under Seal
Birth records under seal by reason of adoption, paternity determination, or court order, must be obtained from the Bureau of Vital Statistics in Jacksonville. - Birth Records in Other States
