Free COVID-19 Vaccinations at Westside High School

Duval County Public School (DCPS) and the Florida Department of Health-Duval (DOH-Duval) are hosting free COVID-19 vaccination clinics on select dates this summer. COVID-19 vaccines are available to anyone 12 years of age and older.
Individuals under the age of 18 may attend without a parent or guardian, but a signed consent form and photo ID are required. Consent forms are attached below.
The clinics will be open from 9:00 AM to 1:00 PM.
Note: The consent form must be filled out completely before receiving the vaccination. See forms below.
DOH-Duval will be providing $10 grocery vouchers to each individual that receives a COVID-19 vaccine. The vouchers can be used at many local grocery stores.
