Back to School Immunizations - Beat the Back to School rush!

DOH-Duval will be partnering with Agape Family Health, Head Start, and Duval County Public Schools to ensure students are prepared for the 2020-2021 school year.

DOH-Duval’s Immunizations Centers will be hosting Saturday walk-in hours from July 11 – August 8, 11:00 a.m. – 3:00 p.m. at South Jacksonville Immunization Center and Pearl Immunization Center.

Clients may call 904-253-1130 to schedule a weekday appointment.

Religious exemptions are only available Monday-Friday at South Jacksonville immunization Center.

 

Agape Family Health will be offering back-to-school immunizations and physicals from July 13 – August 8. Physicals are $30 for those who are uninsured.

Walk-in services are available on the following dates:
