Defeating Diabetes through Education, Awareness and Leadership 

Our Mission: To reduce the mortality and morbidity associated with diabetes and diabetic complications by providing clients with the education, awareness and leadership needed to take control of diabetes and improve their overall quality of life.

  • Follow-up case management services for program participants

How to manage diabetes 

  • Eat a healthy, balanced diet
  • Be physically active every day for at least 30 minutes
  • If you smoke, STOP NOW!
  • Get a flu shot once a year

Tests and examinations needed to control diabetes 

  • Hemoglobin A1C
  • Foot Examination
  • Eye Examination

What is Diabetes? 

Everyone has some sugar in their blood. Diabetes means there is too much sugar in your blood. Too much sugar in your blood can damage your body. Diabetes occurs when the insulin your body naturally makes does not work properly or when your body can't produce enough insulin.

Symptoms of diabetes 

  • Feeling very tired most of the time
  • Passing a lot of urine, especially during the night
  • Feeling thirsty most of the time
  • Having blurry vision
  • Having skin infections or wounds that heal slowly

If untreated, diabetes can lead to:  

  • Loss of vision and blindness
  • Kidney failure
  • Damage to blood vessels leading to heart attack or stroke
  • Foot ulcers and amputations
