Make an Appointment Without Online Access

Individuals 65 years of age and older (who do not have online access) can receive additional assistance with scheduling a vaccination appointment (through the online portal) by contacting Aging True at: (904) 531-5026. Please note: in order to receive appointment assistance from Aging True, individuals 65 years of age and older must be a resident of either Duval County or Clay County.

Only healthcare workers with direct patient contact and individuals 65 years of age and older are able to receive the vaccination at this time.

Information on how other individuals can recieve the vaccination will be available soon.

We are currently working to make the vaccination process easier for individuals 65 years of age and older, which includes identifying vaccination sites that could be more accessible.

New appointment times will become available every Thursday at 5pm. The appointment form can be accessed by clicking on "click here" under "Make an Appointment Online" at the top of this web page.

The current vaccination site is located at the Prime Osborn Convention Center (1000 Water Street, Jacksonville, Florida 32204).
Parking is located on Bay Street (behind the Prime Osborn).
Please bring identification and insurance information to your appointment.

Please wear a facemask and practice social distancing at your appointment.

An appointment for the 2nd dose will be confirmed at the appointment for the 1st dose.

The time between the two doses is as follows:

• Moderna, 28 days.
• Pfizer, 21 days.
