The Florida Department of Health Duval prepares health assessment reports using a collaborative, systematic process to collect, analyze, interpret and use data to inform and mobilize communities, develop priorities, gather resources, and plan actions to improve the public's health. Because it provides essential information for discussion, prioritization and action, a state health assessment process is the foundation for improving and promoting the health of Duval County’s residents.
Subject matter experts from across a diverse group of partners conduct assessments and produce reports and action plans which you can find below. Assessment is an on-going process to address community health problems, improve the quality of care, and identify health inequities. Each year, multiple assessments yield in-depth analysis of factors and forces that impact population health. The assessments taken together contribute to a comprehensive view of health and quality of life in Duval County.
