Health Center Changes - June 2016

Changes have occurred at the Florida Department of Health in Duval County (DOH-Duval) that are important to share with you.

In the past DOH-Duval had received federal funding to operate our community’s Federally Qualified Health Centers (FQHC): West Jacksonville Family Health Center, South Jacksonville Primary Care, South Jacksonville Family Health Center, and the Wesconnett Family Health Center. Under the Public Health Service Act, FQHCs provide core primary care servicesto community memberswithout regard to their ability to pay.

In March 2016, the Agape Community Health Center, Inc. (Agape) received federal funding in the amountof $2.1 millionfor three years to operate the FQHC network independently. Since then DOH-Duval has been providing temporary transitional support to Agape to decrease disruption of access and services to patients.Beginning July 1, 2016 Agape will operate as a stand-alone FQHC.

DOH-Duval will continue to provide services at the following locations:

Central Health Plaza
515 West 6th Street 32206

Services available at this location include:

  • Women’s health (maternity, family planning, annual exams) and pediatrics in the Center for Women and Children

  • STD testing and treatment in the Center for Prevention Services

  • Tuberculosis (TB) Clinic in the Center for Specialized Pulmonary

  • Comprehensive Care Clinic(formerly known as Boulevard Comprehensive Care Clinic)

  • Refugee Health Services

The Beaches Family Health Center

Services available at this location include:

  • Women’s health (maternity, family planning, annual exams) and pediatrics

Magnolia Women’s Health Center
5300 North Pearl Street

(Clients must live in 32202, 32204, 32206, 32208, 32209 and 32254 for Magnolia services)
Services available at this location include:

  • Women’s health (maternity, family planning, annual exams)

Pearl Plaza Immunization Center
5322-24 North Pearl Street 32208

Services available at this location include:

  • Adult immunizations

  • Pediatric immunizations

  • Rabies vaccine

South Jacksonville Immunization Center is now open at 3225 University Boulevard South Suite #101 32216

Services available at this location include:

  • Adult immunizations

  • Pediatric immunizations

Beginning in June, DOH-Duval will begin providing maternity care at our Wesconnett site located at 5150-9 Timuquana Road 32210

DOH-Duval WIC and Dental locations have not changed, including those located at the West Jacksonville and Wesconnett Health Centers.

Agape will offer adult primary care, pediatric, and women’s health services (family planning and annual exams) at:

Agape/West Jacksonville Community Health Center

Additional Agape locations will be announced/confirmed soon.

Community members can schedule appointments for any location (DOH-Duval or Agape) at 253-1130 until June 30. Afterward, only DOH-Duval appointments will be scheduled at this number.
