DOH-DUVAL SAYS “GO TO THE HEAD OF THE CLASS”
FOR BACK TO SCHOOL IMMUNIZATIONS AND PHYSICALS
-Don’t wait until the last minute for mandatory back to school requirements-

JACKSONVILLE - Florida Department of Health in Duval County (DOH-Duval) has launched its “Go to the Head of the Class” campaign to assist parents and families with preparing for the upcoming school year. Parents and guardians should check the immunization status and school physical requirements for their children.  

DOH-Duval Immunizations Center, 5220 North Pearl Street, is offering the following
Back-to-School services:

Walk-in services for all required School Immunizations
Monday – Friday
8:00 a.m. – 4:00 p.m.
1st and 3rd Wednesday
8:00 a.m. – 12:00 noon

Walk –in School Physicals (not sports physicals)
Kindergarten through 12th grade
July 21 – August 27
Tuesdays and Thursdays
8:00 a.m. – 3:00 p.m.
$30 per exam

The Florida Department of Health Bureau of Immunization encourages you to visit their website www.immunizeflorida.com for information about vaccines and school requirements.

Current DOH-Duval parents are encouraged to call 253-1130 to schedule an appointment where their child is usually seen. It is always best to have a complete physical done by the doctor who knows your child.
