Florida Department of Health in Duval County Client Information Breach

JACKSONVILLE -  The Florida Department of Health, Duval County Health Department (DOH-Duval) is issuing a breach notification to certain patients of its Comprehensive Care Center at 515 West 6th Street. Included in the breach were individuals’ names, birth dates, and medical information. There were no Social Security Numbers involved in this incident.  DOH-Duval received notice of the breach on February 25, 2014. An internal review was conducted and notification letters were mailed to all persons involved on March 3, 2014. Because addresses were not current, the Department was not able to contact all patients affected. We at DOH-Duval wish to assure our patients that everything possible is being done to protect their information. The Department is committed to safeguarding confidential patient information. To further safeguard the identity of our clients, the Department has implemented security measures and policies and procedures to restrict and control the availability of patient information.

Comprehensive Care Center clients should review their credit history for any fraudulent or suspicious activities they have not authorized. A free report can be obtained at www.annualcreditreport.com. If you have fraudulent activity, contact the Duval County Sheriff’s Office at 904-630-0500. The Department of Health can answer general questions at 855-456-0647.
